To register onsite, please download the registration form and bring it with you to the summit registration desk at the Statler Hotel. We will not accept any registration forms sent via email or faxed.

*Cancellation, Refund & Substitution Policy

All registration cancellations and refund requests must be made in writing before May 4, 2019. A refund of all fees, minus a $100 administrative fee, will be given for cancellations received by May 27, 2019. No refunds will be granted for requests postmarked after May 27, 2019. Submit all requests to us via email at CMC regrets that refunds will not be given for no-shows. Substitutions are gladly accepted! A transfer of your full registration is permitted prior to the conference by submitting written request to Onsite transfers must be accompanied by proof of the original confirmation email. Badge sharing and splitting are strictly prohibited.

**Students/Faculty Registration
To apply for the student rate you must be a current full time student (12 credits per semester/session.). For faculty rate you must be a full time faculty employee to apply. Provide proof of student/faculty ID. Online registration not available for students and faculty, download the form and send it via email with documentation to
